PROF. DR. MED. DENT JEAN-FRANÇOIS ROULET

Prof. Dr. Med. Dent Jean-François Roulet completed his Dentistry from University of Bern, School of Dentistry, Germany in 1974. He has held various important positions during his long career including Dean, School of Dentistry, Free University of Berlin; he is a visiting Professor, University of Florida and visiting Professor Universita degli Studi di Siena, Facoltà di Medicina e Chirurgia. Since
2003 he has been the Chairman of the Dept. of Research and Development Clinical. Ivoclar Vivadent, Schaan. Prof. Roulet has published more than 400 papers in National and International Journals. His main subjects of interest are:

• Amalgam Substitution - Ceramic Inlays - Aesthetic Dentistry - Preventive Dentistry (application concepts) Since 1972 more than 500 lectures, courses or seminars were given as continuing education for dentistsv in German, English, French and Italian.

HE WILL BE MAKING TWO SEPERATE PRESENTATIONS ANTERIOR ESTHETIC RESTORATIONS ( Fri. 2nd Oct.)

In order to incorporate esthetic restorations in the anterior segment, the dentist must be able to
perform “invisible” restorations. There are several prerequisites to accomplish this goal:

• Knowledge of the basic rules governing esthetics in the oral cavity. Only if the dentist is able to insert an artificial material that looks natural into the oral cavity, there is a chance that the restoration will not be perceived as such by the patients. Knowledge about form, structure, position and colour are determining the dental esthetics.

• Understanding and mastering the adhesive technique is as important as well, because only with adhesive techniques it is possible to camouflage the restoration margins. Therefore the dentist must know how to bond resin based materials to tooth hard tissues (enamel and dentin) as well as to the restorative materials (mainly ceramics)

• Finally good esthetic materials must be available, which show the same optical properties as the teeth themselves. Today excellent composites and ceramics are available which can accomplish these characteristics. To place invisible restorations with direct composite a layering technique must be followed. Basically, dentin is replaced with a more opaque composite mimicking the optical characteristics of dentin. It is then covered with a translucent “enamel” composite in order to create the colour from the depth, as it is done in natural teeth. Basically the same rules govern the esthetics of ceramic reconstructions (veneers and crowns) However these are manufactured by dental technicians and therefore only the important characteristics relevant for the dentist will be discussed. An important step is the cementation. There some specific guidelines must be followed to be successful.

POSTERIOR ESTHETIC RESTORATIONS ( Sat. 3rd Oct.)

In the posterior segment, estetics is not as dominant as in front teeth which are highly visible. Usually the patients are happy, if teeth are “white”. With modern dental materials it is possible to produce naturally looking restorations in the posterior segment as well. Using intelligent layering concepts it is also possible to create even large restorations which are virtually invisible in posterior teeth. The main limitation are the skills of the dentist. The less natural tooth structure is present as a guideline, the more difficult direct restorations get. Furthermore, the possibility to place a matrix band is a prerequisite for direct composite restorations. Indirect restorations may be inlays/onlays or crowns and bridges. There the material selection becomes important, especially if tooth coloured bridges are of concern. Since the ceramic world has become more complicated, the selection of the ceramic especially in combination with the luting technique has become important and crucial for a long term success.

A paradigm shift has taken place in the way we practice dentistry today and hence the dental surgeons are compelled to adopt/ take up modern methods to rehabilitate their patients. But the key to success would be to understand the modality of treatment and analyze every aspect before we undertake the implant surgery for the patients, viz: Preoperative, operative and post operative which in turn helps us to avoid common pitfalls… and achieve greater levels of success. It will be prudent to bear in mind that GET TO KNOW NEW FRIENDS, BUT DON’T GIVE UP THE OLD ONES. 2. LIFE AFTER DEATH......RESTORATION OF ENDODONTICALLY TREATED TEETH..............

The overwhelming success of endodontic therapy has allowed for the retention of more teeth than ever before in the history of dentistry.
In this presentation it will be my endeavor to outline criteria which allow the clinician to identify endodontically treated teeth that can be restored with a high level of predictability. The intent of this presentation is not to preclude the restoration of teeth that do not meet these criteria; it is to provide a science-based approach for identifying those teeth with a high probability of long-term success upon restoration.

For an endodontically treated tooth not requiring a post, the requirements are for biologic width + ferrule length, (i.e. 4.5 mm of supra-bony solid tooth — dentin a minimum of 1 mm thick after preparation). A tooth requiring a post needs, in addition, enough root length to allow a 4 mm apical seal and a post length — apical to the crown margin, equal to the length of the crown. It is essential to assess the functional loads to which the restored tooth would be subjected. This entire workshop has been designed in such a way so as to nable clinicians to take appropriate decisions keeping in mind the clinical situations.

The selection of tooth colored materials used to reconstruct lost tooth structure in the smile zone is mostly based on their ability to copy the color of the natural tooth as closely as possible. Most clinicians would prefer to have a single shade which matches all teeth. However, even the creator of teeth used two different materials to construct the solid entity we know as the tooth. If the visual properties of the tooth have to be mimicked with excellence in mind, then multi-layering becomes mandatory. Despite the best efforts put into Restorative Dentistry, the patient and consequently the clinician faces the oft ignored challenge of maintenance. A dental practice would need to incorporate a preventive protocol for patients if the disease index is to show any improvement over the years. However, the myth of “running out of business”, plagues the mind of the clinician and puts the brakes on active implementation of preventive protocols. This presentation will attempt to close the gap between Restorative and Preventive practices, proposing to highlight
the ease of multilayering of composites for anterior teeth on the one side, and presenting tips for simple preventive protocols which can actually function as a separate profit centre, breaking down the above mentioned myth.

tissues during office bleach procedures using a high percentage of hydrogen peroxide would

Abstract: Bio-medical waste is defined as solid, liquid or sharp waste that is
generated from health-care (medical) activities such as diagnosis, monitoring,
treatment, immunization of human beings or prevention of disease or
alleviation of handicap in humans or animals, including related biomedical
research and are contaminated with patients body fluids (such as syringes, needles, ampoules,
organs and body parts, placenta, dressings, disposables plastics and microbiological
wastes). Proper disposal of hospital waste is of paramount importance because of its infectious
and hazardous characteristics. Proper handling of biomedical waste is crucial to the
Public Health and needs to be convened with standard operating procedures. The Govt. of
India promulgated the Bio-medical Waste (Management & Handling) Rules, 1998 in trying to
ensure that proper handling & management of hospital wastes should be implemented in all
the hospitals and clinics including dental clinics.
