Guided Implant Surgery

Prosthetically driven implant placement, with a perfect three dimensional position, is the key to success of any implant case. With modern technologies, 3D radiographic aids and proprietary softwares which are available today, clinicians can virtually plan positions of implants prior to surgery. Final tooth position and important anatomic landmarks are given utmost importance during the planning process. Additionally, with the prevalence of the desktop 3D printer, we are now able to fabricate affordable 3D printed surgical guides. With the help of advanced and sophisticated guided surgery instrumentation and armamentarium, we are able to ensure placement of the implant at the planned 3 dimensional position.

The instructor will discuss case planning steps and perform a live demonstration of implant placement in the patient.

The Direct Way To Natural Aesthetic

The loss of natural tooth structure calls for restorative procedures to replenish the missing tooth substrate in form, function & aesthetics.

There has been a tremendous evolution in the composite materials available to us today and in the varied innovative techniques for their clinical usage. Due to this, the clinical applications of direct composites have increased manifold in routine restorative practice.

A stepwise morphologic layering technique which is based on stacking materials with varying degrees of opacity and translucency, can be a highly effective method to create optically biomimetic restorations. This live case procedure performed by Dr. Prashant Hatkar is designed to demonstrate the stepwise build-up, finishing and polishing of a class 4 multi-layered direct composite restoration, which remains to be a challenge for the clinician to simulate the natural tooth in terms of shape, shade and texture.

Lasers In Dentistry

Are lasers truly exclusive in their use?

Do they do something which conventional dentistry does not do? Do lasers aid in getting better results in conventional dentistry?

Is it time to get into laser dentistry or should we wait some more? Over the last decade I had been chasing this wonderful science because of its exclusive features.

Target specificity – the ability to select what you want to strike and do no damage to the adjacent tissue.

The need for anaesthesia has dropped to just topical application – NO SHOTS!

No need for antibiotic cover during the surgical procedures!

No need for the patient to discontinue his/her regular medication!

Bio stimulation – this was the deciding factor which was the clincher to buy the first diode laser – phenomenal healing!

Relief from muscle spasms etc etc.

The hard tissue laser and its unique characteristics gave us the ability to prep teeth without anesthesia – Painless dentistry!

Increased bond strengths with our composite restorations!

Not to mention the unique ability to sterilize the root canal space with laser energy and its ability to rapidly heal large periapical lesions!

Let’s catch up and discuss more during the presentation.

Contemporary Protocols in the Clinical Management of Early Childhood Caries

A Contemporary Dental Practice should aim at applying the principles of evidence- based dentistry to achieve excellence in preventive and treatment outcomes. The understanding of dental caries has witnessed revolutionary changes in the recent past. This has influenced the prevention and management of early childhood caries significantly. Current knowledge of carious tissue removal when applied to clinical practice can be of immense benefit to our pediatric patients. These minimally invasive techniques greatly reduce the need for aggressive stress inducing clinical procedures. Approaching the management of early childhood caries in this new way can transform the pediatric dental experience for both the little children we treat and for us.

Tips & Tricks To Master Single Sitting Pulpectomy, LSTR And Intentional Reimplanatation.

It is indeed the happiest moment of your dental practice when you can perform Pulpectomy in the tiny mouth of a child patient in a Single Sitting using Single File. Performing Single Sitting Pulpectomy offers numerous advantages and reduces the number of re-visits required, thus reduced discomfort. Several factors hold importance while performing a Single Sitting Pulpectomy with successful clinical outcome. Appropriate case selection with well-planned clinical approaches fostered by the skill and experience of the operator and most importantly correct selection of obturating material are cornerstones of successful pulpectomies. Selection of file for biomechanical preparation remains an important task when you perform pulpectomy in single
visit. Young permanent teeth although may appear similar to other permanent teeth, require special consideration and modifications in treatment planning. Application of LSTR technique is a boon in management of teeth with poor prognosis and aids in preservation of baby teeth. This lecture will highlight all the important aspects that need to be considered in order to achieve success in Single Sitting Pulpectomy.

How To... Decide When To Cement, When To Bond?

Today more than ever we are spoilt for choice when planning and delivering indirect restoration. From the moment our patient comes in, we have much to consider before deciding on the treatment plan. As a Dentist we know that no two patients are alike and it is therefore important to follow a holistic approach in treatment planning considering the choice of indirect restoration and type of preparation which is best suited to the patient’s clinical situation. This paradigm shift in consideration of indirect restorations has also effected the choice of luting solutions. The conventional luting protocols also have undergone a paradigm shift. The ‘HOW TO” session focuses on luting of various prosthesis of various substrates by understanding “ when to bond & when to cement “ theory and helps in redefining the protocols of luting.

TIILT - A Paradigm Shift For Maxillary Implant Patients

As we all know that Maxilla specially posterior maxilla has a lower success rate than mandible
because of the Quality as well as Quantity of bone which happens due to the pneumatisation of maxillary sinus as well as tendency of maxilla to go into class 3 post extractions.
So managing maxillary edentulism is sometimes a challenge for the dental practitioner, especially if patient wants immediate loading also . So we have modified the existing approach of All-on-4 and expanded our horizons to other bones which are cortical in nature and have a very less chance for resorption like Nasal, Pterygoid and Zygomatic bones . So with this new concept we can almost for every maxillary case place and load implants immediately within 24 hours.

Dental Ergonomics

BE A PAIN-FREE DENTIST….POSTURE MATTERS “AN OUNCE OF PREVENTION IS WORTH A POUND OF CURE”

Dentists are at high risk for musculoskeletal disorders due to the nature of their work. The key to preventing work-related musculoskeletal disorders is ergonomics — the science of fitting the work environment to the worker. In dentistry, ergonomics involves the design, adjustment and modification of operatory layouts, counters, delivery systems, stools, instruments and patient chairs to minimize excessive reaching, twisting, leaning, gripping and repetitive motions.

Literature has shown that more than 65 % of Dentists suffer from musculoskeletal complaints varying in severity but accompanied with pain, discomfort, hindrance in functioning and loss of working time. Our goal with ErgoDontia is to improve Dentist performance and reduce occupational pain by focusing on proper body mechanics and preventive exercise to safeguard against injuries.

Pterygoid Implants Or Sinus Lift? Which Is Better For Your Loved Ones?

IMMEDIATE IMPLANTATION AND LOADING WITH SIX IMPLANTS WITH TTPHIL ALL- TILT™ TECHNIQUE.

In the edentulous upper jaw the placement of implants can be challenging due to limited bone quantity and the presence of the maxillary sinus. Pterygoid implants present an alternative option to use residual bone for implant anchorage and to overcome the need for augmentation procedures. To avoid distal cantilevers the insertion of a pterygoid implant can be indicated in partial and total edentulism.

PTERYGOID IMPLANTS OR SINUS LIFT?

Bone in Pterygoid area is cortical in nature and hence very strong and rigid allowing immediate loading with permanent crown in few days. Very high success rate. Engaging the cortical bone which is highly resistant to resorption. Eliminates the need of sinus grafting. Bone grafting procedures are contraindicated. No cantilevers from front teeth or implants- WIDE AP SPREAD. Efficient chewing is possible as teeth upto second molars are replaced. In cantilevers implant cases or in all-on -4 and all – on – 6 techniques 2nd molars are not replaced. Flapless technique is used, as such patients have no swelling or bleeding and patients need not stop blood thinners, if they are taking any. Very useful when posterior teeth are missing.

Clear Aligners: Expanding The Scope Of Orthodontic Care

Advances in clear aligner technology have changed the way orthodontics is practiced today. The use of clear aligners as a means of sequentially aligning abnormally positioned teeth has evolved significantly over the last few years. Once limited to correcting mild orthodontic problems in fully erupted dentitions, this treatment modality is now being used by experienced Clinicians in a broad spectrum of the comprehensive treatment of complex malocclusions. In spite of the acceptance and usage of clear aligners by the Clinicians growing significantly every year, many still need to recognize the complexities involved in this method of treatment.
This presentation deals with the essential components of the aligner therapy, and highlights the efficient workflow involving diagnostic criteria, treatment planning process, digital set-up, and clinical management of aligners to achieve predicted treatment goals in diverse clinical situations.

The Importance Of Oral Hygiene And Maintenance For Longevity Of Implants

The success of an implant is not just dependent on the surgical procedure and subsequent delivery of the prosthesis. In the long term the implant is prone to the same problems as a natural tooth as regards oral hygiene. Peri Implantitis and its precursor Mucositis are issues that impact the implant when oral hygiene is not maintained. There are a number of long term studies that have shown how poor oral hygiene can impact the longevity of an implant. This presentation will deal with all the necessary protocols involved in the long term success of implants and also how infected implants can be restored back to health and then subsequently maintained.

Management Of Complications In Curved Canals

The Biomechanical preparation of curved root canals is an important consideration in endodontic treatment. In addition to the endodontic instruments and root canal preparation techniques, root canal morphology and the degree of curvature are determining factors in endodontic root canal preparation.

The deformation of endodontic instruments placed in a curved canal creates stress on the instruments.

Tensile stress is graduated on the non-curved parts and compressive stress occurs on the curved part of the root canal instruments. The morphology of curved root canal is of great importance to the outcome of the root canal treatment.

The chances of iatrogenic errors are increased in such cases. It is to be particularly taken care of, which sailing through the curves of canal. Special precaution and techniques are used, as recommended, shaping of curved canals without any complication is possible in accordance with its anatomy.

Indirect Restoration Within Minutes- Simply Chair Side

Composite restorations is the mainstay of restorative dentistry and the problems associated with the restorations can be catastrophic. The long term success of the same is dependent purely on the operator and the technique. Most common issues with composite restorations are that related to marginal leakage and improper isolation techniques. If only a predictable solution to these issues can be used then the marginal leakage and the post-operative sensitivity problems can be limited. Chair side inlay technique is one such simple protocol which will limit the problems associated with the lack of isolation in the oral cavity.

Digital dentistry getting popular and softwares trying to reduce human error in planning and execution gave more predictability and sustainability of Dental Implants in oral rehabilitation. Preservation of remaining and regaining the lost hard and soft tissues to get aesthetic outcome for implant supported prosthesis is the thought of this digital era. Basic concepts of ideal position planning and three dimensional placement of a dental implant is very crucial in achieving optimal results. Prosthetically driven Implant placement, planning the correct size, width and length of an implant matters a lot. Wide and short implants in posterior rehabilitation and its advantages and its ideal positioning, planning the prosthesis using CBCT and guided surgery, are topics discussed.

Immediate Implant - Where Are We Now

Rehabilitating complete or partially edentulous patients with Dental Implants has been an acceptable treatment modality. Dental Implants in the aesthetic zone has been a challenge, both in healed sites and extraction sockets. Aesthetic is one of the evaluation criteria’s for the success of the implants has lead to changes in the treatment methods. Immediate implants have been successful treatment options with various techniques proposed by various Clinicians. Minimizing soft tissue changes following extraction of teeth and Implant placement is been a challenge in modern Implant dentistry. Gingival recession will not only effect aesthetics but also the long term survival of the implant. This presentation will discuss all the techniques performed and how we have predictably addressed the challenge and where are we now.

Occlusal Splint Therapy

Stability of any structure depends on co-ordination and balance between its components, same way stability of masticatory apparatus depends on coordination and balance between temporomandibular joint, muscles, ligaments and teeth. Loss of co-ordination among any of these components leads to occlusal problems and TMDs e.g. Myofascial pain (MPDS), clicking in the joints, pain in jaw, head, neck, shoulder and ears often misdiagnosed as migraine, tension headache, spondylitis, etc. Depending on symptoms and awareness patient goes to respective medical speciality and patient get only symptomatic relief and runs around speciality to speciality in the search of cure. But most of the time problems lies in occlusion and loss of neuromuscular control, so we need some treatment which will brings co-ordination among masticatory apparatus and help to reduce pain of patients thus giving diagnosis, same way helpful in formulating final treatment plan In this lecture I am going to highlight role of OCCLUSAL SPLINT THERAPY in the diagnosis and treatment of TMDs and occlusal problems which causes pain in jaw, head, neck, shoulder and ear.

Clinical Markers in Endodontics

Current concepts in root canal system preparation still largely rely on mechanical & chemical preparation of root canal system. Two major factors affect the choice of instruments for root canal preparation, its cleaning and shaping ability and safety with complete disinfection of root canal treatment with intracanal medicaments against the microbial flora. For more than 25 years, NiTi have been used to shape root canal space. Ever since companies have been trying to improve endodontic files in order to achieve a more predictable outcome of the shaping procedures and enhance the cleaning efficiency of endodontic irrigants and intracanal medicaments & fracture free instrumentation along with magnification which will able to achieve predictable endodontics. Innovations focused on the designs of the files such as cross sections, taper, helical angles and many other specifications that distinguish to identity of a specific instruments and the efficacy of intracanal medicaments to achieve predictable success in infected root canal on healing of periapical lesion and weeping canals. This evolution reduced significantly file breakage & increased antibacterial action of calcium hydroxide that nevertheless was still haunting NiTi users and pushed forward the researches into new direction.

The Final Word: Screw V/S Cement Retained Restorations

Implant dentistry has revolutionized the way we treat our patients for the past two decades. While the concept behind implant dentistry is fairly simple, the multitude of options available in managing an implant prosthetically is not entirely straightforward. At the most fundamental level, fixed implant prosthesis can either be screw retained or cement retained. However, the question remains as to which one is truly the superior option, both for the practitioner and the patient. This tandem lecture will to go into detail regarding all of the pros and cons of both systems, backed by the latest scientific evidence and will guide the viewer into making a more informed decision in choosing the correct type of prosthesis.

Laser V/S Conventional Periodontics

Technology has changed the way we live our lives. A desk containing a computer, calendar, phone, files and other accessories is now replaced by a device we like to call a “smart phone”. But when it comes to dentistry, how smart are we
in recognizing the need for technology, adopting it and ensuring we know its pitfalls along with its advantages. This session will help you decide whether LASER has replaced the blade in Periodontics and if you should adopt it.

CPD V/S Flexible Partial Denture

Achieving success with Removable Partial Dentures could be challenging because of the unfavorable dynamic forces acting intraorally. A definitive treatment option could be using cast metal frameworks in the form of CPDs, where surveyed models improve the stability and retention of the prosthesis, thus improving acceptability by the patients. But is cast partial denture an option for periodontally weakened dentition? An option for collapsed verticals?
Are our patients ready for expenses associated with cast frameworks and precision attachments on unpredictable dentition? Do flexible dentures offer a quick fix until some definitive option can be provided? Would patients adjust to problems related to Nylon dentures? Have newer flexible bases really evolved for a wider application? All this and more at our comprehensive debate on the myths of rehabilitating partially edentulous arches using removable prosthesis.

    Bioline Pterygoid Implants Tilt Technique

    Dr. P.Venkat Nag

Immediate implantation and loading with six implants TTPHIL ALL TILT*TM PROTOCOL: Placement of implants in the maxilla can be a strenuous task owing to the positioning of the maxillary sinus and limited bone density. Sinus Lift and other conventional techniques have been widely approved methods for a long time but with the shortcomings in these techniques, never advances have come into practice. Pterygoid implants present themselves as a novel approach almost revolutionising implant dentistry. Pterygoid implants enjoy a higher success rate, thanks to their strategic placement in the pterygoid area providing immediate loading. This area being cortical, proves to be more durable even making it possible to provide permanent crowns in a very short span. The cortical bone exhibits minimal resorption enabling higher stability.WIDE - AP SPREAD - mean no cantilevers from the front teeth or implants while in conventional techniques likes all-on-4,all-on-6 ,the second molars are not replaced. This being a flapless technique, leads to minimal bleeding and swelling in a patient making it a more comfortable procedure. Patients under prescription anti-coagulants do not have to stop their medication for the procedure making this technique highly recommended and useful when rehabilitating missing posterior teeth.
