Abstract

In recent years Endodontic treatments have increased a lot because of increased healthcare counselling and patient awareness towards preserving the natural tooth.

Also it is a fact that with higher numbers of Endodontic treatments there are chances of Endodontic mishaps like perforations, missed canals, instrument separation, improper obturation, re-treatment, etc.

The Fear: The biggest fear for all of us is to encounter Endodontic mishaps like perforations, missed canals, instrumentseparation, etc.

The Challenge: The challenge for us is to overcome the fear of Endodontic mishaps and looking forward to think in the direction how to manage such situations.

Abstract

Modern Endodontics has given better prognosis to the teeth since year 1996. But sometimes clinicians get carried away during some procedures. Fractures or cracks have become major reason for endodontic failures. Its time to decide that to achieve straight line access, one has to balance the conservation of tooth structure. We have seen that ideal straight line access may lead to lot of destruction of the tooth. This may lead to facture of the tooth under heavy masticatory loads. Follow up of some teeth which are treated by greater taper preparations show cracks in the teeth after 2 to 3 years.

Differential diagnosis of these cracks could be traumatic bite, selection of the post and destruction of the tooth structure during access preparation and selection of larger taper in shaping.This lecture will elaborate the factors influencing access cavity preparation and how to achieve straight line access with a MIE and which taper one should opt for.

It is time to evaluate the conservation of the tooth structure during endodontic therapy to improve longetivity of the tooth.

Knowledge on the causes of and the mechanisms behind interappointment pain in endodontics is of utmost importance for the clinician to properly prevent or manage this undesirable condition. Pain and/or swelling after instrumentation of a root canal constitute a significant complication during endodontic treatment. Despite a large number of studies discussing the causative factors behind endodontic flare-ups, the exact mechanism is still not understood. This lecture will focus on the mechanisms of interappointment pain, with special emphasis placed on the causative agents and the host response to injury that can precipitate pain. In addition, diagnostic measures and treatment approaches to manage interappointment pain will also be discussed, followed by by a good 3D obturation and post Endodontic restoration.

Abstract

Full mouth rehabilitation of a severely worn dentition is considered as one of the most challenging procedures in dentistry. Until recently, the only predictable way to occlusally restore a compromised dentition was through full coverage crowns and bridges. Although this protocol has served many patients for decades, the very thought of stripping the tooth of its enamel in order to get retention and resistance form (from an already worn out dentition) just never seemed right to me. Fortunately, we clinicians live in the era of ‘predictable enamel bonding’ that eliminates the need for mass enamel murder and retentive preparations.

Adhesive bonded restorations for anterior teeth in the form of laminates and veneers have been an established treatment protocol in dentistry for long. Taking this very tissue-preserving protocol to posterior teeth, adhesive bonded restorations like inlays, onlays, vonlays/vtops, overlays, crownlays, endocrowns, etc. have been practiced with immense success across the globe. The main advantage of this approach is its minimally invasive nature that aims at retaining the enamel bio-rim which allows the tooth to maintain its core strength.

This novel biomimetic approach of restorative dentistry can be carried to the world of full mouth reconstruction through the use of glass ceramic materials and even composites over occlusal surfaces of posterior teeth to restore lost tooth structure (VD), a protocol famously referred to as ‘TableTop Dentistry’. By combining the traditional principles of physiologic centric position with the modern bio-emulating principles of minimally invasive dentistry, full mouth reconstruction patients can benefit massively from predictable long term function and aesthetics without elaborate cutting of tooth material.

Yes, the protocols are meticulous and have a learning curve involved. However once mastered, this leak-proof bonded lifestyle is by far the best form of dentistry to offer our patients. Over the last 13 years she has worked as a Senior Lecturer and Assistant Professor in Dental Colleges and a Consultant in various clinics all over Mumbai. After a successful career in Cosmetic Dentistry she entered into the world of Facial Aesthetics in 2013. Being a dentist she had an aesthetic eye well suited to evaluate the symmetry and the aesthetics of facial expression. Since then extended her knowledge in this field by completing an International Masters Course in Skin Laser therapy from Singapore and Certification in Aesthetic Medicine from Hong Kong. Presently practicing as an experienced Facial Aesthetistician, carrying out Facial Rejuvenation involving Botulinum toxins (such as Botox) and Dermafillers. Also conducts lectures and workshops on Facial Aesthetics on a regular basis. She loves how simple and subtle changes to smooth and relax muscles can “freshen” a face to look younger. So its time to “think outside the mouth”.

Abstract

From the shoulders up, every aspect of the head and neck affect a person’s aesthetics and self-confidence. More specifically, the hard and soft tissues of the middle and lower third of the face contribute, to a large or small extent, to dentofacial aesthetics and they are all interrelated. Today, dentists and cosmetic physicians are changing the manner in which they correct issues affecting dental and facial esthetics and embracing a more comprehensive dento-facial approach. A combination of cosmetic dentistry, injectable fillers and neurotoxins & Rejuvenation with PRF is now being used.

This lecture introduces the use of Botulinum Toxin for dental indications like gummy smiles, massetric hypertrophy as well as cosmetic purposes like frown lines, crows feet, forehead lines, as well as use of derma fillers for correction of Nasio labial folds, lips, chin and cheek enhancements. Dermal Fillers can be used intra orally for black triangles. Also introducing the use of Smart Blood Derivatives – Platelet Rich Fibrin (PRF) for skin & hair Rejuvenation.

Institute of Dental Sciences, K.L.E. University, Belgaum as a Senior Lecturer in Department Of Conservative Dentistry and Endodontics. He is a co-author of two bestselling entrance books in India – Dentest by Gowrishankar and Dental Bytes. He has also cleared NBDE, ADAT for Dentistry in USA and currently runs a dental institute-Aspire32. His articles in the field of Endodontics have been published in multiple national and international journals. He is a key speaker at Famdent Shows, World Dental Show and various other IDA conferences and a guest speaker for many Continued Dental Education programs. Bangladesh Academy of Dentistry International (BADI) recently invited him as a guest faculty for a lecture on Endodontics.
